The basis of the ACA is that companies are required to provide coverage that is affordable. Determining exactly what that means, however, can be a challenge.
Retirees enrolled in any of UC’s self-insured plans receive this form from UC. IRS Form 1095 is intended to give you information about your medical coverage, who was covered, and when. You may receive more than one form, however, which can be confusing. Here’s an overview of the three versions of IRS Form 1095 so you’ll know what to look for, and why you may receive each form. Form 1095-C is filed and furnished to any employee of an Applicable Large Employers member who is a full-time employee for one or more months of the calendar. ALE members must report that information for all twelve months of the calendar year for each employee.

Moulder expects the IRS will release draft instructions in the coming weeks that will provide additional details—and perhaps greater clarity—about completing the revised Form 1095-C. The IRS also released a draft of 2020 Form 1094-C, the transmittal form that accompanies Form 1095-C when filed with the IRS. If you were a member of Health Net or Kaiser in 2020, reach out to your health plan for reprints or corrections to Form 1095-B. Payroll Information
This form is informational only — it shows whether you were offered health insurance by King County last year. The W-2 reports employees’ wage and salary information as well as the amount of federal, state and other taxes withheld from their paychecks. Keep a copy of the form with your tax records for future reference. If you have any questions about the information contained on the 1095-C form, please contact the issuer. Though many companies have completed their first year of ACA annual reporting, penalty notices have not started. Self-insured companies with more than 50 employees, then, are actually required to send covered workers both a 1095-B and a 1095-C.

Does Your Company Need To File Form 1095
Controlled groups exist when any two or more entities are treated as a single employer under section 414 , , , or of the Internal Revenue Code. Where the combined total of full-time and FTE employees in a controlled group is 50 or more, each individual employer is subject to reporting with respect to its own common law employees . Access Your Form 1095

The IRS uses the information from Form 1095-C to administer the Employer Shared Responsibility provision. The Form also helps the IRS administer premium tax credits for any employee who qualified and enrolled for coverage at a Health Insurance Marketplace rather than enrolling in your employer plan.
